ACCESS TO MEDICAL RECORDS
The access to health record (AHR) Act came into being in 1992 allowing people to have sight of their medical records Under the data protection act 1998 you have a legal right to access all of your health records including those of your GP. If you require a copy of a result/letter etc contact the surgery and you will be asked to complete a consent form to release this information; which can usually be done within 48 hours.
If you wish to have sight of your entire medical records this may take longer. You will be asked to sign a consent form to release this information. If you wish to just see your records, a time and date will be arranged with you that is mutually convenient. (We have to ensure that a room is available for this so days may be restricted.) If you wish to have a complete copy of your records this may take some time but we will try our best to arrange this within one week of the request. As your records will have to be photocopied a small administration fee may be applied; you will be notified of the cost prior to them being copied.
If you need access to any health records that are held by any community based site including the following hospitals: George Eliot Hospital or University Hospital Coventry and Warwick Or any of the following Services: Health Visitors
You can request the relevant forms from NHS Warwickshire by writing to: AHR / FOI Officer Westgate House Market Street Warwick CV34 4DE
